Early Explorers recognizes
that children need to be healthy and have proper nutrition in order
to learn and grow to his or her full potential. Preventive health
services are the center of the program's comprehensive services.
Early Explorers ensures that all enrolled children have timely immunizations
and screenings for cognitive development, basic health, speech and
language, hearing, vision, and dental health. Services are coordinated
to assist each enrolled family to create and maintain a positive
relationship with a medical
home. The program helps families track well-child visits and
immunizations and provide anticipatory guidance regarding infant
and child health, nutrition and development. The program provides
ongoing health education for the entire family using newsletters,
handouts, home visits and parent trainings. There is a stong emphasis
placed on promotion of wellness, nutrition and prevention.
Staff work with parents
and health care professionals to determine if the child is up-to-date
on a schedule of age-appropriate preventative and primary care health
services. This schedule must incorporate the requirements of well-child
care utilized by the Early and Periodic Screening, Diagnosis and
Treatment (EPSDT/Health Tracks) the Medicaid program of the State
in which the program operates. Programs also must establish procedures
to track the provision of health care services. They provide important
information and are encouraged to participate in health promotion
activities. Community representatives are also involved in EHS/HS
program planning through participation on the Health Services Advisory
Committee, which comprises parents and professionals from the community.
